What documents are required upon admission?Upon admission to the facility please bring the following: All insurance cards including your traditional red, white, & blue Medicare card, Advance Directives you may have filled out prior to admission including Power of Attorney for Healthcare and Life Sustaining Treatment form.

How are medications ordered?Medications are provided from Oregon Pharmacy where we order our medications through.

Can I have my own phone?Patient's are encouraged to bring their own cell phone or family can call Frontier to have a line installed into their loved one's room.

What type of activities are available?We have a wide range of organized activities for all skill levels. Our monthly activity calendar promotes independence and enhances quality of life. We encourage families to come in and partake in activities with their loved ones.

Can we bring in medications from home?No. We do not allow medications to be brought in from home. They have to be provided by our pharmacy.

Do you have Wi-Fi?Yes! We offer a guest wi-fi. All you have to do is click guest and sign into the browser.

Are beauty and barber services available?Beauty and barber services are available upon request or on a regular schedule with our beautician. Patients can request services through the activities or admissions department. Payments can be given ahead of time at the billing office or it can be added to the patient's bill and billed to them with their monthly fees. Patients can also pay from their trust account if they have one set up at the time of admission.

What type of services do you offer?We offer Short Term Rehabilitation, Long Term Care, Respite Care, and Outpatient Therapy

Is there transportation available to patients?Patients can be transported by our staff for an additional cost. Please reach out to the billing department for the exact cost.

What is provided in the room?We provide a hospital bed, TV, Dresser, Closet, Chair, and Nightstand in each room. If you have a chair or TV from home you would like to bring in please let us know in advance. Thank you.

What are your different care levels?We offer: Short Term Rehab- patients who are recovering from a surgery, illness, or who need extensive wound care can qualify for short term rehab. Long Term Care- patients who are no longer able to live at home alone and need 24 hour care. Respite Care- patients who's caregivers or loved ones are in need of a break from providing care can qualify for a respite stay.

Is Laundry included?Yes, laundry is included however if family would prefer to do their loved ones laundry they are more than welcome to.

Who qualifies for short term rehab?A patient with Medicare part A has to have a 3 midnight qualifying stay in the hospital and have a need for skilled services. A patient with Medicare Advantage plans have to have prior authorization prior to leaving the hospital.

What type of rooms are available?We offer private and semi private rooms.

Do we need to provide any linens?Patients do not need to provide their own linens of any sort. We provide bed linens and towels. If you would like to bring your own pillow, you are welcome to do so.

Do you have a locked dementia unit?At Medina we do not have a locked Dementia unit. However, we look at each patient individually and can accommodate most dementia patients. If we cannot meet a patient's needs we will inform you prior to accepting a patient.

What is included in the daily room fee?Room rates include: room and board, basic nursing care, three meals a day, snacks, housekeeping services, laundry services, activities, social services, and maintenance services.

Can patients have cash?At the time of admission patients are given the opportunity to open a trust account with us to use at their discretion.

What insurances do you accept?We currently accept Medicare, Medicare Advantage plans, Managed Medicaid Plans, Medicaid, VA contracts, and many community health insurance plans. We check your insurance prior to admission for a short term rehab stay or for long term care if coming in under Medicaid or VA contract.

Can we have a meal with our loved one?Yes, you can buy a ticket in advance at the business office. We will set you and your loved one up in an area different from the dining room with our size it just isn't feasible inside the dining room.

Are there set visiting hours?There are no set visiting hours at Medina. We allow loved ones to come see patients any time they would like.

What is included in Short Term Rehab Stays?We provide 24/7 nursing care, advanced medical supervision, rehab therapists (Occupational, Physical, and Speech), pain management, wound care, nutritional support, and more.
